JOB SUMMARY The Outreach Worker/Interpreter acts as a resource and advocate for the migrant and seasonal farm worker population by canvassing camps and work sites, assessing health needs, promoting services at the medical and dental sites, and providing or arranging for interpretation services for non-English speaking patients. Participates in a team-based approach to care in the Patient Centered Medical/Specialty Home (PCMH/PCSH).Participates in activities related to Quality Improvement.
This is a seasonal position from May 1st - Nov. 15th.
ESSENTIAL JOB DUTIES Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. - Canvasses migrant farm worker camps, other residences and work places to distribute information about the health center and mobile units' services and hours.
- As trained, provides patient education and performs health screenings such as blood pressure, pulse, temperature and weight while at the camps
- Identifies clients in need of health and/or social services and makes appropriate referrals.
- Under direction of the medical clinic staff, monitors patients who have chronic or acute medical conditions and encourages compliance or assists with treatment regimes.
- Joins the mobile travel teams when in the area and assists and interprets as needed.
- Completes and submits all required documentation in the patient's record in a timely manner.
- Acts as liaison between local Health Center and migrant patients.
- Assists in clinic set-up, closure and cleaning procedures.

JOB SPECIFICATIONS - Education: High School Graduate or GED required.
- Licensure: N/A
- Experience: Previous experience with farmworker population preferred.
- Skills: Able to communicate effectively with a diverse population. Bilingual (English/Spanish). Must be proficient in both English and Spanish.Must be able to read and write proficiently in both languages.
- Interpersonal Able to work effectively with limited direct supervision.
Skills: Flexible in scheduling priorities and accepting assignments.
Able to function effectively as member of a care team.
Ability to provide services in an empathetic, non-judgmental manner. Comfortable providing services in patients' homes.
Physical Effort: Medium
Hours of Work: 10-12 hours per week, mostly late afternoon evening hours. Monday through Friday, or as needed, to complete the job.Flexible and varied.
Travel: Extensive travel is required within service area.Minimal out-of-service area travel may be required to attend conferences/trainings. Reimbursement for expenses provided according to GLBHC policy.Must be a licensed driver with proof of personal liability and property damage insurance.
